The policy describes the protections for keeping personal individual information (PII) confidential while allowing for reasonable, responsible, and limited uses and disclosures of data.

This policy describes standards for the security of personal information collected and stored in the HMIS and elsewhere in print or electronic formats within the Alameda County CoC network.

This version of the notice is for organizations providing services to homeless clients that have adopted the Alameda County policy to customize and post where clients can see it.

The alternative notice is for HIPAA agencies and other service providers that want a signed ROI. The U.S. Department of Housing and Urban Development (HUD) suggested this language in the Federal Register/Vol. 69. No. 146/Friday, July 30, 2004/Notices SEC. 4.2.1 pg. 45929.

This is a consent form used for housing and homeless services that allows for the client’s personal individual information (PII) to be shared with other service providers that assist clients. This form is required for any use or disclosure that is not listed in the agency’s privacy notice. Some organizations may require that this form be collected on all clients.

Clients sign this form to revoke their consent to share data outside of the HMIS covered network. Any changes will take effect as of the date the form is signed, and any data or information shared before that time cannot be recalled.

Staff of service provider agencies use this form to formally confirm that they reviewed the Privacy Notice with the client, offered assistance with reading the Notice, and gave the client an opportunity to ask questions.

The manual describes the roles and responsibilities of key players who use, oversee, and administer the HMIS, the processes for bringing in new agencies to participate in the HMIS, user training and support, and how the HMIS privacy and security policies are to be implemented.
